Please arrive 15 minutes before your scheduled appointment to fill out paperwork. Your initial visit will last approximately 40 minutes. We’ll discuss your medical history, any specific concerns and your goals for rehabilitation.
What do I need to bring to my appointments?
Please bring the following to each appointment: Your photo ID; Detailed history of symptoms and treatments; Full list of medications with dosages, including over-the-counter supplements; Comfortable clothing you can easily move in; List of questions for your care team.
Do I need a referral?
No, you don’t need a referral before scheduling an appointment with us. Walk-ins are also welcome.
