Do I need a physician referral for my physical therapy appointment?

For many insurance companies, a physician referral is not required for a physical therapy appointment. However, referrals are required for Medicare and Medicaid. We recommend first consulting with your insurance company before calling to schedule an appointment. If you have a referral, you can fax it to us at (828) 274-6101.

What do I bring to my first appointment?

When you visit us for the first time, bring: Insurance and/or Medicare/Medicaid card, Photo ID, List of current medications/supplements, Physician order if provided by physician/MD office, Registration form, Medical history form, Attendance policy, Consent for treatment (English/Spanish).
