Will insurance cover my labs?
Yes! However, we always encourage you to check your lab coverage through your insurance provider. Restore Functional Medicine offers discounted lab services that may be a more affordable than billing your insurance. Feel free to call us for more detailed information.
Do you accept insurance?
We are a cash practice and are not contracted with insurance carriers. Patients may request a receipt for services that they can submit to their insurance company for potential reimbursement. We are able to accept HSA, credit card, debit card, cash and personal checks for services. Patients with insurance are able to use their coverage for labs, referrals, imaging and other testing ordered by our office. This allows us to remain affordable for primary care patients who will only pay cash for the services rendered in our office.
What is your cancellation policy?
If you are unable to keep your appointment, please provide 24 hour notice and we can assist you with rescheduling your appointment. Failure to provide 24 hour notice results in a fee of $100.
