What are the eligibility requirements for The Dental Clinic?
The Dental Clinic provides basic services at reasonable cost to people without private dental insurance and whose family income falls below 250% of the national poverty guideline. Patients must live in Allen, Barren, Butler, Edmonson, Hart, Logan, Metcalfe, Monroe, Simpson or Warren counties. Patients are asked to provide a copy of their current Federal Tax 1040 form on first visit, or a Federal Form 4506-T (which we will provide) to help us qualify you for eligibility.
