What is the focus of the Geriatric Clinic?

Age-specific concerns and treatment within geriatrics; non-urgent geriatric consultation.

A referral from your provider.

Pre-visit requirements

The following forms must be completed and returned before the first appointment. You may return the forms via email to SeniorHealth@OhioHealth.com, within MyChart, or fax the forms to (614) 566-1916. Forms include: Pre-visit questionnaire; Family Exchange of Authorization Form; Physician Exchange of Information Authorization.

Is a referral required to schedule a visit?

A referral from your provider is required before scheduling services at Gerlach.

What happens during the initial appointments?

An initial telephone conversation with a clinical social worker to identify concerns and discuss matters most important to you, followed by a medical examination with a geriatric physician, a registered nurse and a licensed social worker (appointment typically ~2 hours). Bring all medications, glasses, hearing aids, and ambulatory devices.

What services are provided?

Non-urgent geriatric consultation services; assessment results inform treatment plans with your primary care providers. Team specializes in aging issues and can provide information and referrals to community services.

What should I bring to the exam?

Prescription medications, over-the-counter medications, vitamins and supplements, glasses, hearing aids, and ambulatory devices (cane, walker, wheelchair). Wheelchairs are available on site.
