Do you take health insurance?
No. We do not take health insurance and do not bill health insurance. We provide covered primary care services through the membership and special discounted pricing for medications and labs. Individuals are welcome to use their health insurance outside of Harris Family Health, if applicable. Dr Harris is out of network for health insurances.
How do I start a membership?
Complete the enrollment form, review the membership agreement, pay the first membership fee, then schedule a new patient appointment.
What is included in the membership?
The membership covers primary care services, preventative screenings, sick or acute care visits, simple procedures, chronic care management, and access to discounted medications, labs, and imaging services.
How do I sign up for a membership?
You can start your membership by completing the enrollment form here. You will then be emailed a membership invitation to review the membership agreement and pay the first membership fee. Once enrollment is complete you will receive information on making a new patient appointment. Monthly membership payments will start being charged to your payment method on file the next month per the agreement.
Can I cancel my membership?
Yes. Memberships can be cancelled at anytime with a 30 day notice but a re-enrollment fee may be charged when you return. Membership services will immediately end at cancellation and membership fees are non-refundable.
