MedCentris Home to Heal® brings wound care directly to patients’ homes for those who cannot travel.

Who is eligible for Home to Heal®?

Homebound patients or those with significant limitations in accessing outpatient wound care, including seniors, mobility-impaired individuals, and residents of rural areas.

How are services delivered?

Wound care is administered by trained MedCentris clinicians who come to the patient’s home, delivering services based on the individual’s plan of care.

How is care scheduled?

Appointments are scheduled based on the patient’s condition and treatment needs, ranging from multiple visits per week to less frequent follow-ups.
