Are new patient forms available before visits?

Adult, pediatric and behavioral health new patient forms are available.

What are the new patient forms and how should I complete them?

Prior to your first visit, please fill out the following forms for adults, pediatrics, and behavioral health; if you would like to complete the forms in the office, please arrive 15 minutes early.

Do I need to bring anything or arrive early for my appointment?

Please arrive 15 minutes early for new patient visits.

What is the co-payment policy?

Your co-payment is due in full at the time of service; exceptions exist such as annual physicals and injections. If you have no insurance, payment is due at the time of service.

What payment methods are accepted?

We accept cash, check, money orders, Visa, MasterCard, American Express and Discover. Returned checks are subject to a $25 NSF fee.

What is the cancellation policy?

24 hours notice of cancellation is required if you are unable to keep your appointment; a minimum of one hour’s notice is required for same-day appointments. After three failed appointments without proper notice, you will be dismissed from the practice.

Do you handle referrals and insurance authorizations?

Many insurance companies require referrals; authorizations should be in place prior to care. Most referrals take 72 hours to process, but it can take longer, and approval is not guaranteed.

Do you offer well-child checks and pediatric services?

Yes, the practice provides full pediatric care including well-baby and well-child checks.
