Who is eligible for dental clinic services?

Lincoln and Lancaster county residents. Federal income guidelines determine eligibility and payment level; income is based on household size and total income. Proof of income is required.

Can I still get services if I cannot pay the minimum fee?

No family or individual will be denied dental services if unable to pay the minimum fee or sliding fee at the time of appointment, but payment is expected; payment plans are accepted.

What documents are required for eligibility?

Proof of income according to Federal guidelines; forms must be updated annually. Complete one per household and one per patient; bring current medications list.

Is there a form I need to print before visiting?

Yes. Print forms, fill with black ink, fax to 402-441-8142 or bring to appointment; a child health history must be signed by a parent/guardian.

Are there special forms for pregnant clients?

Yes. An Authorization to Treat (Pregnant Client) form must be signed by the physician; bring the completed form to the appointment.

Who can bring a child to the appointment if the parent cannot attend?

Complete the designated form to allow another person aged 19 or older to bring the child. The parent/guardian must sign authorization.

What about multilingual support or interpretation?

What age range is covered for children?

For children between the ages of 1 and 18.

Do services cover adults on Medicaid?

Yes, for adults covered through Medicaid.

Do services cover pregnant women?

Yes, for low-income pregnant women without dental insurance, and/or presumed eligible for Medicaid.
