What to bring to your first appointment?

Driver’s license or other photo ID, Social Security Card if applicable, Insurance ID cards, Medicare/Medi-Cal card(s) if applicable, registration form.

1. Driver’s license, valid Passport, or other valid photo identification 2. Social Security Card, if applicable 3. Insurance ID card(s) showing group & policy number, if applicable 4. Medicare and/or Medi-Cal card(s), if applicable 5. Registration form printed and completed (see below)

What should I do if I need an interpreter?

Yes. Before your appointment, you can request a list of languages for which professional interpreter services are available by calling (619) 662-4100. All Patient Services Representatives can help with scheduling an interpreter.

Are you a free clinic?

San Ysidro Health is a Federally Qualified Health Center offering high-quality, affordable healthcare. If you have government-issued medical insurance (e.g., Medi-Cal), you may qualify for medical care at no cost. If you do not have government-issued medical insurance, you can apply for our sliding scale fee discount program or a self-pay plan.

How can I access the sliding scale discount program?

The discount is based on family size and income. To apply, provide proof of income (e.g., most recent paycheck stub). A complete list of qualifying documents is available from a Patient Services Representative at the clinic’s registration desk. The discount is valid for up to six months and can be renewed.
