Will Health Care Connect immediately guarantee a match?
No. Those who need urgent care will be placed first, and a match may take time.
What information is needed to sign up?
Your name, address, health card number and some basic health information to help determine health needs.
What happens after a match is found?
The found family doctor or nurse practitioner will contact you with their information; you may choose whether to make them your clinician, or Health Care Connect will continue searching if you decline.
