DO I NEED A DOCTOR'S REFERRAL TO START PT?
California allows direct access to physical therapy services. You can see us DIRECTLY without a referral for 12 visits, or 45 days (whichever comes first). After that, your PT will coordinate with your doctor if more visits are needed.
DO YOU ACCEPT MY HEALTH INSURANCE?
Being an out-of-network practice, we don’t directly accept health insurance. However, your insurance carrier may reimburse you a portion of what you paid. The amount depends on your specific plan and insurance carrier.
HOW DO I SUBMIT A REIMBURSEMENT CLAIM TO MY INSURANCE?
For most insurance providers, you can log on to your insurance plan online. Search for “out of network claims.” You will need to complete a claim form to submit with your Jet Physio superbill (invoice receipt that we provide for you). You can typically submit this online, or by mail. If you are having trouble, please call your insurance carrier on the back of your card.
