How do I make an appointment?

Returning patients - If you have seen your provider within the last three years, please schedule an appointment online through MyChart. If you have additional questions, you can call the existing patient number at the top of this page. New patients - Please call the phone number at the top of this page to request an appointment.

What do I need to bring to my child’s first visit?

Please bring the following to your child’s first visit: Sports Physical Form from Washington Interscholastic Activities Association (WIAA), Your photo ID and insurance card, Co-payment, if required by your insurance plan, Full list of medications with dosages, including over-the-counter supplements, Immunization records, Detailed history of symptoms and treatments, List of questions for their care team.

What if my child has an urgent medical need?

If your child has an urgent medical need and our office is closed, please visit one of our urgent care centers.

What about emergencies?

For emergencies, please call 911.
