What should I bring to my first appointment?

Please plan to arrive at least 15 minutes before your appointment, 30 minutes is even better. When you arrive, there will be forms that you need to complete before your session can begin. Bring a form of ID (adult clients) and a form of payment. Also bring any reports from healthcare professionals or schools. Unless advised otherwise, children who will be receiving treatment need to be at the first session.

Do I need a referral from my medical doctor to see a therapist at New Horizons Wellness?

No, you don’t need a referral. However, for occupational therapy, if you plan to submit claims to insurance for reimbursement, your insurance may require that you have a referral. However, if another provider or agency is recommending that you get treatment, it is always helpful to have that referral information. For most people, this involves a simple request to your primary care physician’s office. If you have concerns about getting a doctor’s referral, please let our Patient Access Specialists know and we will do what we can to help find the best option.

I’m a referral coordinator/healthcare practitioner, and I have a client I’d like to refer. How can I do that?
