How do I start receiving services?

1. Call us to start the process. If you have an emergent need, please call us at (575) 461-0099 or our on call number at (575) 403-8680. 2. Complete a Referral. Anyone can make a referral. Once the referral has been completed, you can expect to hear back from us within one business day. In order to be admitted, the patient must have a physician’s order. We are more than happy to reach out to the patient’s health care provider in order to get the necessary information to initiate services. 3. Have an Initial Visit. At this visit, the patient and caregivers will work with our admissions team to complete an assessment, order medications and begin the patient’s personalized plan of care.

Who can refer a patient for hospice?

Anyone can make a referral.

What happens after referral?

We will contact the patient’s health care provider to obtain the necessary information to initiate services and you can expect to hear back within one business day.
