Appointments | What should I bring to my first appointment?

For your first appointment, plan to arrive 10-15 minutes before your scheduled time. Bring your insurance card(s), State ID, a list of medications you are taking, and if your doctor ordered therapy, bring the referral or prescription.

Insurance | Do you accept Workers Compensation?

Yes! Each of our locations specialize in treating patients with work-related injuries and we are able to see any patient with an approved workers compensation claim. Our office will assist with contacting your workers compensation carrier to ensure that your claim is open and billable.
