How should I obtain a CD of my imaging results?
Call Medical Records or visit the Switchboard to request a CD; you must fill out a release form and provide a valid driver’s license. Only the patient can pick up their CD unless a release form lists another authorized person.

Please bring your driver’s license, insurance card, and your physician’s order when you arrive at the outpatient registration area.
