New Patients: What to Bring to Your Appointment
If you are a new patient, please arrive 15 minutes early to complete the necessary patient information forms. It is requested that all new patients bring the following to their first appointment: insurance card, state-issued photo identification, referral forms, if required, co-payment, if required, copies of pertinent diagnostic tests and medical records, list of medications with dosages or the original prescription bottles, completed history and physical form, if available. To ensure optimal care, please provide us with hospital records or results of GI testing at another facility. Please have copies of the reports sent to our office prior to your first visit.
