Class schedule and attendance requirements for DSME (diabetes self-management education)
Classes are held in the Hózhǫ́ógo Iiná Wellness Center and organized by HIWP and led by Family Nurse Practitioner & Clinical Provider, Lita Scott and clinical staff. DSME is held every 2nd & 4th Wednesdays of each month. Class begins at 8 am to 1 pm; to attend, the patient's Provider must send a referral to the Wellness Program or the Program can also reach out to a Provider for a referral.
Diabetes Shoe Clinic eligibility
Shoes are provided through the Diabetes Shoe Clinic. A referral is needed to receive a pair of shoes offered through the SDPI grant at no cost to the patient. A patient is eligible to receive 1 pair of shoes a year, and receive shoe inserts up to 3 a year.
