Call to request a medical release form. A signed, completed release form is required, and a fee may apply.

We accept most major insurance plans. Check your policy to confirm coverage; all copays, coinsurance and deductibles are due at the time of service.

What is the cancellation policy?

Notify us no later than 24 hours in advance if you need to cancel; missed appointments without notice may result in a fee.

What should I do if I need a prescription refilled?

Call your pharmacy about refills. Refills are subject to provider approval; refills for controlled substances require an appointment every 90 days.
