What insurances do you accept?
We accept most major insurance plans. Check your policy to confirm coverage so we can submit the claim correctly. All insurance copays, coinsurance and deductibles are due at the time of service.
What is your cancellation policy?
We respect your time and strive to keep wait times to a minimum. To assist us in providing timely care, please notify us no later than 24 hours in advance if you need to cancel your appointment. Missed appointments without notice may result in a fee.
What should I do if I need a prescription refilled?
Please call your pharmacy about any medications requiring refills. All medication refills are subject to provider approval. Refills for controlled substances require an appointment every 90 days and cannot be filled outside of a scheduled visit.
