Telemedicine Information
In an effort to keep our patients and health care workers safe, we are offering virtual visits and phone visits. This service is available to both new and existing patients. Patients should call 870-572-6507 to schedule a telemedicine appointment.
First visit procedures
Please fill out and bring all new patient forms with you to the clinic for your first visit. We will also need a copy of your driver’s license (or another photo ID) and a copy of your insurance cards.
Check-in requirements
For your first appointment, please check in 15 minutes before your appointment time. We will need to verify your insurance and collect co-pays/deductibles at check-in.
Prescription refills
If you need a prescription refilled, call your pharmacy and allow 48 hours to process. If we need to see you before we can refill, we will call you.
Opioid treatment program
If you feel you need a narcotic, we will provide information on our opioid treatment program. You will sign an Opioid Treatment Agreement and urine samples may be required.
After-hours assistance
If you or a family member need assistance outside of regular hours, call 870 972-0063 to speak with an on-call provider.
