Please arrive at least 15 minutes prior to appointment to allow for adequate time for registration, check in, etc.
If you are a new patient and you are able to print and bring with you the completed registration form it will be an easier and quicker process. If you are not in the office, with all necessary paperwork completed by your appointment time you will need to reschedule for another day/time. Thank you for understanding. This policy helps us ensure that each patient gets the fully allotted time of their appointment.
