What is required for a determination to be completed?
All programs require filing an application and submitting program specific verification. Eligibility is based on income and resources. All applicants must submit a Supplement A application. If you are not already receiving Medicaid through the local Department of Social Services, all applicants must fill out an Access NY application as well.
What are the 2026 income and resource limits for Chronic Care Medicaid?
Resource for a Single Individual: $32,396.00; Resources for a Community Spouse: $74,820.00. Income level for 1 person: $1,800.00/monthly (Short-Term stay, Temporary stay and Assisted Living); Income level for 2 people: $2,433.00/monthly (Community Medicaid); Community Spouse Income Allowance: $4,066.50 (Skilled Nursing, PACE and Hospice Residential). Spousal budgeting may be used in some instances; please contact the Chronic Care Department for additional information.
