What are the steps for scheduling or preparing for an appointment?

1. Call us at (801) 277-2682. 2. Arrive 5 - 10 minutes early for your appointment. 3. Bring your current insurance card with you to each appointment. 4. Remember to bring your co-payment, deductible, or co-insurance amounts as required by your insurance plan.

What if I don’t have insurance?

If you do not have insurance, please be prepared to pay for your visit at the time services are provided.

Billing questions

We welcome calls regarding bills and insurance, M-Th 8am - 4pm, and Friday 8am - 1pm. You may call the phone number listed on the bottom of your monthly statement, or call (801) 277-2682, and you will be connected with a billing consultant who will assist you.
