What documents do I need to bring to my first appointment?

Bringing proof of income to your first appointment is required to qualify for services. Proof of income can be various things, such as a Medicare or Medicaid card, insurance card, two pay stubs from employment, child support, TANF letter, etc.

What forms explain your rights and allow release of information?

These forms explain your rights when you receive Behavioral Health treatment and authorize the release of protected healthcare information and can be used to request that a copy of your treatment records be sent to another agency. It is best not to sign these forms until you come to our clinic so staff can witness your signature.
